An employee was cleaning out chunks of wet PVC resin from the side of a mixing machine. While he was attempting to remove the bolted-on steel plate, the plate (which was under pressure) blew out and struck the employee on the right hand and dorsal forearm. He sustained three broken fingers on his right hand, a torn-off fingernail, and lacerations to his right thumb and left dorsal forearm. | |||||

A Delaware City Refining Company employee was walking to his work location on 1/23/16, at approximately 1:00 AM during an intense snow event. As he was walking, he stopped on the side of the road to allow a snow-plowing pickup truck to pass and then crossed the street. The pickup truck reversed and its back bumper contacted the employee from behind, fracturing his femur. He was hospitalized. | |||||

An operator was injured while preparing equipment for maintenance by flushing the equipment with condensate (warm water) and flushing it into an oily water sewer system. While performing the flush, it went to the hydrocarbon sewer where it flashed and ignited. An employee tripped and fell to the ground. The employee landed on his knees in a pile of hot petroleum coke. The employee sustained burns on both legs from the coke, and possibly the hot steam. The employee was taken by ambulance to Christiana Hospital, and later transferred to Crozer-Chester Medical Center in Chester, Pennsylvania. The employee sustained 2nd degree burns on his legs, and 3rd degree burns on his ankles. |
